Tutor Request Form
Items with (*) are required.
Read the following:
1. Tutorial assistance is dependent on the availability of tutors.
2. Student-athletes may be paired with other student-athletes, specifically in the cases of having the same class.
3. Student-athletes will be notified via email about their tutor. He/she has one week to contact the assigned tutor to set up meeting dates/times.
4. All tutoring sessions must be done in the Academic Center. All other sessions must have approval from the Academic Support Manager.
5. If you are assigned a tutor and fail to meet with your tutor without making prior arrangements, the following will occur:
First time – warning
Second time – Must pay the cost of the tutor ($11.00)
Third time – Must pay the cost of the tutor ($11.00) + Lose priority to use tutor
Note: The tutor will only wait for the first 15 minutes after the arranged date/time.
6. If at any time the student-athlete decides that he/she does not need future tutoring, the student-athlete must contact both the tutor and Steve O’Brien (554-7846).
7. All student-athletes must follow the student code of conduct in all interactions with their tutor.
Additionally:
If you receive a tutor through the Academic Resource Center, you will be required to do the following in order to maintain the use of your tutor:
1. Upon receiving an email with the tutor’s contact information, you will be expected to contact the tutor and determine a mutually agreeable time to meet. If your schedules are in conflict with one another, let the Academic Support Manager know, and we will find you another tutor.
2. If you are in need of more than 2 hours of tutoring per week (total for all classes), you must obtain prior approval from of the Academic Support Manager.
3. Submit a brief description to the Academic Support Manager on a monthly basis including the following information:
a. The subject in which you are being tutored and the specific class.
b. The subject matter that you are presently covering.
c. A signed statement indicating that you:
i. Have attended every class with the exception of those classes for which you have team competitions.
ii. Have attended your professor’s office hours at least every other week and that you have taken some additional steps to improve your standing in that particular course (i.e. requesting sample essays/questions, performing additional problem sets, participating in study-groups)
