Independent Contractor Form (MS Word) This form must be filled out if your Network is paying a speaker or independent contractor for their services. Review the Independent Contractor Guidelines (MS Word) prior to filling out the form.

Sample Budget (Excel) Use as template for Network specific budget
Financial Forms

Network Deposit Report (Excel) This form must be filled out if your Network has any deposits to submit

Network Reimbursement Request (Excel) This form must be filled out if your network has any reimbursements to clear

Sponsorship Form (PDF) This must be used to submit sponsorship money
Event Forms

Event Request Form (Excel) This form must be completed one month before an event may be scheduled. Please e-mail the completed form to GBPStudentLife@scu.edu
