• Application form
• Application fee
• Official transcripts from all institutions attended
• Graduate admissions test scores
• Personal statement
• Letters of recommendation
• Any financial aid forms for that school

Most programs will not review your application until all parts of it are submitted. Start preparing the application components early and send them in as soon as possible.
