You MUST complete the survey once you have started it, as your work will NOT be saved to complete at a later date.  Please allow yourself enough time to complete it.  Depending on how much you have to say, the survey can take anywhere from five to thirty minutes.

These evaluations are taken very seriously, as they are used in faculty evaluations and for requests to the Dean’s office for more resources.  Please take your time and provide a careful and honest assessment.

This survey is anonymous. Your name and ID# will not be associated with the rest of the survey; we ask for them only to ensure that you have completed the form before we sign off on your graduation petition. The results of the survey will not be released to the faculty until after graduation.
