Level I (freshman) application form
- Students may apply to Level II after completing 32 quarter units at Santa Clara or an equivalent four-year institution.
- Obtain an application form for Level II from the UHP office.
- Along with (a) the form itself and (b) the personal statement it describes, applicants should (c) request two recommendations from university instructors and (d) arrange an interview with the Program director.
Applications should be submitted during the first week of spring quarter.
