FSA Plan Year 2009 Timeline Reminders:

1. November 30th is the last day to turn in your enrollment for the 2010 plan year. I have attached the form below for your convenience. This may be returned to either our Benson or Loyola locations.  Should you have any questions please feel free to contact Caroline Zelaya (x5750), Herbert Kan(x4097) or Anne Mota (x1934).
2. You have until midnight on December 31st 2009 to purchase eligible items for reimbursement for your current medical FSA account.
3. You must turn in any receipts directly to TLC for the 2009 plan year by March 31st 2010.

* If you would like to view your current 2009 plan year elections you may do so by logging into your ecampus account and view your voluntary deductions under the payroll option. The annual election will be listed under Goal Amount.
