Dear Students,

We wanted to inform you of our hours as we approach the end of this academic year. We will be closing Friday, June 13th at noon and will remain closed until Monday, August 18th.

Should you need your medical records transferred to another health care provider, you can download the medical release form from our website and fax it to the Health Center at 408-554-2376. Your medical records are kept for 7 years after your last visit at the Health Center.
