Health Benefits Waiver Credit for 2012

Greetings SCU Community,
As mentioned in the ʹAnnouncing Open Enrollmentʹ email sent on Wednesday, October 26, 2011 as of January 1, 2012, employees who have other medical coverage available and choose not to enroll in the SCU medical plans will be able to elect to receive a healthcare waiver credit of $150 per month ($75 per pay period). All employees who are either currently waived for 2011 and those who choose to waive out of medical coverage for 2012 must complete a Health Care Waiver Form and provide proof of other insurance* in order to receive the credit. All forms must be received by the Department of Human Resources by November 30th 2011. If you need additional time in providing the proof of other coverage due to your spouse or registered domestic partnersʹ open enrollment period, please let me know.

*Proof of other insurance can be a copy of your insurance card, a letter from your spouse or registered domestic partnersʹ human resources department or a copy of your 2012 enrollment form for that other insurance.
Frequently Asked Questions
May I still have dental though SCU and be eligible for the credit?
Yes, the credit is for waiving out of the medical and bundled vision plans only. You will may keep all other plans, dental, FSA, Life...etc
If my spouse ʹ s/registered domestic partners ʹ insurance is also with SCU do I need to provide proof?
When will I see the credit on my check?
January 2, 2012 paycheck.
If I ʹ m already waived out of the medical insurance, do I need to complete the form and provide proof of insurance.
