Enrollment

Enrollment Requirements

In order to be eligible for Kids On Campus, a child:

  1. Must be at least 6 weeks old at the time he or she begins KOC.
  2. Must be registered for full-time.

Kids On Campus does not discriminate on the basis of ethnicity, gender, religion, or physical capability.

Enrolling Your Child

To enroll at KOC you need to provide the membership secretary with the following:

  1. $50 non-refundable registration fee
  2. Registration form
  3. Enrollment Agreement
  4. Release of Liability, Informed Consent, Assumption of Risks and Indemnity Agreement
  5. Talent Release
  6. Consent for Medical Treatment
  7. Physician's Report (current within the last six months)
  8. Pre-admission Health History
  9. Parent's Report
  10. Emergency Information and Consent Form
  11. Identification and Emergency Information - Day Care Centers
  12. Notification of Parent's Rights
  13. Personal Rights - Community Care Facilities and Child Day Care Facilities
  14. Statement Pursuant to Penal Code Section 11166.5 (Child Abuse Reporting)
  15. Parent Class Schedule (students only) where you can be reached at any time.
  16. Infant and Toddler Needs and Service Plan.
  17. Toddler Toilet Training Plan

All of these forms must be returned to the Director before your child may begin attending Kids on Campus.

Immunizations

In addition to a signed physician's form, evidence of updated immunizations must be on file at KOC:

Needs and Services Plan

Infants' and toddlers' needs change rapidly. You will be asked to update your child's Needs and Services Plan quarterly so that we know of any changes.

Your child needs the following at KOC (clearly labeled with name):

Infant Room:

  • Formula or breast milk fully prepared in bottles clearly labeled with your child's name.
  • Disposable diapers (KOC will provide wipes).
  • Baby food/special meals. Please label all containers with the child's name and the date. We will discard leftovers at the end of each day.
  • Two complete changes of clothes to be left at the center.
  • Blanket and two port-a-crib sheets.

Special Blankets, Cuddles, and Pacifiers
Most children are comforted by meaningful objects at home and they are encouraged to bring something to the center that has a special meaning. Please make sure that these items are labeled and taken home (if appropriate) to be laundered.

1 to 2 Year Old Room & 2 to 3 Year Old Room

Each child has a cubby for their personal treasures and their art work.
Each cubby will include:

  • 2 complete change of clothes (including underwear, socks, and shoes)
  • Full-size crib sheet
  • Blanket and small pillow (with a cover that can be washed). Bedding is to be taken home for laundering EVERY WEEK on Friday and returned on Monday.

If your child will be at KOC for lunchtime, please send a nutritious lunch (sandwich, fruit, crackers, cheese, soup in a thermos, microwaveable foods, etc.) PLEASE DO NOT SEND CANDY OR GUM TO SCHOOL!

Please do not send foods that could cause choking, such as:

  • Peanuts and/or whole nuts
  • Whole grapes
  • Honey
  • Fruit with seeds and or tough skins not peeled
  • Popcorn
  • Chunks of hot dogs

Milk is provided by KOC, please do not send juice boxes in your child's lunch. Lunch boxes or bags must be labeled with your child's name. Lunch is a time for sharing and learning social skills -- please feel free to join us for lunch anytime!

Do not leave medicine or vitamins in your child's lunch box!

Breakfast foods are allowed prior to 8:45 a.m., if brought to school ready to eat or prepared by parents at KOC. Please bring breakfast milk from home. We keep just enough milk on hand to serve at lunch.

With the exception of special "sharing" toys and/or "napping buddies," do NOT send toys to school with your child. They may be lost or broken and sometimes the source of disputes.

Children should be dressed in comfortable play clothing. Hightop shoes, overalls and belts, for example, are often difficult for young children to take off and on. For safety's sake we ask that no thongs or slippers be worn to school.
