On-Campus Passport Day

- CANCELED - Applying for a U.S. Passport requires in-person appearance at a federally-approved processing center. Why make the trip to foggy San Francisco to stand in line at a processing center when you can do it right on campus? For one day, the Williman Room will be an approved processing center for students, faculty, staff, their families and friends, and people in the neighborhood around campus. Co-sponsored by the Office of International Programs and Kelly Harada from the Mailing Services, this session will bring a team of U.S. Postal Service clerks to campus to process and accept U.S Passport application documents.  

Step 1: Sign Up Online for Appointment

We need a minimum of 100 registered participants by November 5, 2011 in order to bring the clerks to campus. Please click here or scroll to the bottom of this page for our online Registration Form to register yourself and/or your family members who will participate in this session. We will contact you via email to confirm your actual appointment time.

 

Step 2: Review & Complete Required Forms

Visit the Department of State website to read the instructions for "First Time Applicants": http://travel.state.gov/passport/get/first/first_830.html and complete  the DS-11 form and print a copy. 


Step 3: Bring Supporting Documents to Your Appointment

Compile all other required documents as indicated on the State Department's website to bring to your appointment at Santa Clara. These required documents may include the following:

  • Printed copy of Form DS-11: Application for a U.S. Passport. (Do NOT sign until instructed by the clerk during your appointment)
  • Evidence of U.S.Citizenship (see website for acceptable documents)
  • Proof of Identity and Age (Driver’s license or government issued ID)
  • Photocopy of the Identification Document you will present
  • Passport Application Fee (see FAQ below)
  • Execution Fee  (See FAQ below)
  • Two Passport-size photos (get them done on campus, see below for details)

WHAT ARE THE PASSPORT FEES?


Regular Application Processing Fees:

$110.00  - Application Fee (check/money order payable to "Department of State")
$25.00 - Acceptance Fee check/money order payable to "USPS"

Expedited Processing Fees:

$182.72 - Application Fee (check/money order payable to "Department of State")
$25.00  - Acceptance Fee (check/money order payable to "USPS")
$18.30  - US Express Mail Service Postage Fee (check/money order payable to USPS")


Other passport fees are…

 

Minor (under 16) – $80 with an acceptance fee of $25

Passport card – add $15 for minors, $30 for adults

Adult renewal – $110

To renew your passport, you must be 16 or older and eligible to use Form DS-82.

Form of Payment?
The Acceptance Fee of $25 can be in the form of cash, check (personal, certified, cashiers, travelers), a money order (U.S. Postal Service®, international, currency exchange), or a credit or debit card made payable to "USPS" at the time of your appointment.

Application and any Optional Service Fees:
The Application Fees and any other Optional Services Fees must be paide with a check (personal, certified, cashiers, travelers) or money order, made payable to “Department of State.”


CAN I SUBMIT APPLICATIONS FOR MY KIDS UNDER AGE 16?:

Please check the State Department's website for the appropriate form for minors under age 16. Both parents or legal guardians must appear in person to sign the forms unless you complete the appropriate Power of Attorney forms found on the State Department's website. Please make sure you complete the appropriate forms or the clerks will not be able to process your children's applications.

 

WHERE CAN I GET MY PASSPORT SIZE PHOTOS DONE?

You can get them done right here at the Office of International Programs to raise funds for the International Club's activity fund and for a global scholarship to send impoverished high school students in the Pacific islands to attend Catholic and Jesuit high schools. COST: $10 for 2 photos; $17 for 4 photos - CASH ONLY). Photos can be done Mon-Thur from 9:30 - 4:30 PM.


HOW LONG ARE THE APPOINTMENTS?

Once we reach the minimum number of registered participants, then we will contact you by email with appointment slot. Each appointment will be approximately 5 minutes IF all your documents are complete and in order. Please plan for up to an hour just in case people ahead of you cause delays in the processing time.
