On-Campus U.S. Passport Application Day

Co-sponsored by the International Programs Office and University Mailing Services, this session brings a team of U.S. Passport clerks and a passport photographer to the Santa Clara campus to process and accept U.S Passport application documents. This session is open to all students, faculty, staff, and their families. If you want to participate in this workshop, you must RSVP to secure a spot in one of the appointment slots.

REQUIRED DOCUMENTS:
Participants are required to bring the following documents to the session:

(1) Completed Passport Application  https://pptform.state.gov/FraudAbuseNotice.aspx?form=DS11

(A computer terminal will be available on site, but it is highly recommended that participants complete the form prior to the session)

(2) Original Birth Certificate

(3) Proof of Identity and Age (Driver’s license or government issued ID)

(4) Passport Application Fee of $67 and Execution Fee of $30; $97 total; cash or check only

(5) Two Passport photos
(A passport photographer will be available on site for $15. Please arrive 10-15 minutes prior to your appointment time to have your photo taken OR you can get passport-size photos from Costco or other photo processing centers)

FOR APPOINTMENT: Space is limited so secure a spot by calling (408) 554-6940 or come by the International Programs Office located at the Cesar Chavez Commons, A (trailers next to O'Connor) to schedule an appointment. Deadline to make an appointment is November 5th.
