&lt;ol&gt;
    &lt;li&gt;November 30th is the last day to turn in your enrollment for the 2010 plan year. I have attached the form below for your convenience. This may be returned to either our Benson or Loyola locations.&amp;nbsp; Should you have any questions please feel free to contact Caroline Zelaya (x5750), Herbert Kan(x4097) or Anne Mota (x1934).&lt;/li&gt;
    &lt;li&gt;You have until midnight on December 31st 2009 to purchase eligible items for reimbursement for your current medical FSA account.&lt;/li&gt;
    &lt;li&gt;You must turn in any receipts directly to TLC for the 2009 plan year by March 31st 2010.&lt;/li&gt;
