Blue Shield cards and PCP ID's

Employees who enrolled in any of the Blue Shield plans will receive their member cards by January 1, 2019.

Employees who enrolled in any of the Blue Shield plans will receive their member cards by January 1, 2019. When you receive your member card, please verify that your Primary Care Physician (PCP) ID is listed correctly.

HMO plan members and dependents who either didn’t provide a PCP or provided an invalid number for their PCP information will be randomly assigned a primary care provider.

Starting January 1, 2019, you can contact Blue Shield to update the PCP. The contact phone numbers are as follows:

  • (855) 829-3566  for TRIO HMO Plan
  • (855) 599-2650 for PPO and Access Plus HMO Plans

Note:

  • Members can update their PCP in the first month (1/1-1/31) of coverage retro to 1/1 if you do not see the assigned PCP first.
  • Members either in the first month of coverage (1/1-1/31) who have seen the PCP or beginning 2/1 will need to wait until the first of the following month for their new PCP to kick in.
