We write to share that, on May 18, 2021, the Santa Clara County Public Health Office ordered that all businesses and governmental entities in Santa Clara County must ascertain the COVID-19 vaccination status of all of their employees, contractors, and volunteers regardless of on-site or remote work status. This reporting is ongoing and SCU is required to update this information every two weeks. For detailed information about this order, please visit this link, section 9.c. SCU is required to comply by June 1, 2021, or face penalties of up to $5,000 per day of non-compliance.
This County requirement is unrelated to the work of the UCC appointed SCU vaccine working group and the ongoing discussion related to requiring SCU employees to be vaccinated.
Based on this new County requirement, you must visit this link and respond to this quick survey no later than June 1, 2021. If you respond to this survey that you are not yet fully vaccinated, or you decline to answer, you will be asked to complete this survey every two weeks or until you are fully vaccinated. Additionally, employees that decline to provide vaccination information will be recorded as unvaccinated.

As announced, SCU will be hosting a free, on-campus COVID-19 vaccination clinic Wednesday, June 2 from 10 a.m. to 5:30 p.m. You may also visit the Santa Clara County website to secure a vaccination appointment. Please visit the SCU Vaccine FAQ site to learn more about safety and efficacy. Getting vaccinated is the best thing you can do to keep yourself safe from COVID-19 and prevent you from spreading COVID-19 to your family, friends, coworkers, and neighbors.

Q. Why am I being asked to provide this information?
A. On May 18, 2021, Santa Clara County issued an order that requires SCU to ascertain and report the vaccination status of its employees by June 1, 2021.
Q. Will I be asked for this information again?
A. If you respond to this survey that you are not yet fully vaccinated, or you decline to answer, you will be asked to complete this survey every two weeks or until you are fully vaccinated.
Q. I already registered my vaccination in Benson Memorial Center (Vax Verify). Do I need to reply to this survey?
A. Yes. We need to have every employee respond to this survey. Once you confirm that you’re fully vaccinated, you will not receive additional requests to complete this survey. Please note that we may require confirmation of vaccination status again as we return to campus for fall 2021. In addition, as noted in the County order, SCU may request documentation of your vaccination status, (e.g., a copy of your vaccine card or other similar official document confirming vaccination status).
Q. What will be done with this information?
A. Employee vaccination information is generally treated as confidential by the university. SCU is required to have this information available to demonstrate our compliance with the Order. In addition, as dictated by the County and/or Cal/OSHA, this information will be used to provide guidance on the health and safety protocols for vaccinated and unvaccinated employees which will be communicated as State regulations become final.
Q. If I am a JST employee working in Alameda County, do I have to complete the survey?
A. Yes, all SCU employees must complete the survey.
Q. If I am working remotely from a location outside of Santa Clara County, do I have to complete the survey?
A. Yes, all SCU employees must complete the survey.
