Infant/Toddler Enrollment

Enrollment Requirements

In order to be eligible for Kids On Campus, a child:

  1. Must be at least 6 weeks and 2 years old
  2. Must be registered for full-time

Enrolling Your Child

To enroll at KOC you need to provide the director with the following:

  1. $100 non-refundable registration fee
  2. Registration form
  3. Enrollment Agreement
  4. Release of Liability, Informed Consent, Assumption of Risks and Indemnity Agreement
  5. Talent Release
  6. Consent for Medical Treatment
  7. Physician's Report (current within the last six months)
  8. Pre-admission Health History
  9. Emergency Information and Consent Form
  10. Identification and Emergency Information - Day Care Centers
  11. Notification of Parent's Rights
  12. Personal Rights - Community Care Facilities and Child Day Care Facilities
  13. Parent Class Schedule (students only) where you can be reached at any time.
  14. Infant and Toddler Needs and Service Plan (Star Fish only).
  15. Toddler Toilet Training Plan (Star Fish only).

Immunizations

In addition to a signed physician's form, evidence of updated immunizations must be on file at KOC:

  1. DTP (Diphtheria, tetanus, Pertussis)
  2. MMR (Measles, Mumps, Rubella) and Polio
  3. HIB (Meningitis) vaccine
  4. TB Test (Tuberculosis)
  5. Hepatitis B
  6. Varicella
  7. A signed physician’s report form

First Day of School - What do I Bring?

Your child needs the following at Kids on Campus (clearly labeled with name):

  1. Formula or breast milk fully prepared in bottles clearly labeled with your child’s name.
  2. Once your child starts drinking regular milk, please bring in a small carton.
  3. Disposable diapers (Kids on Campus will provide wipes)
  4. Baby food/special meals. Please label all containers with your child’s name and the date. We will discard leftovers at the end of the day.
  5. Two complete changes of clothes to be left at the center.
  6. Blanket and two port-a-crib sheets. (You are responsible for cleaning these as necessary- bedding taken home on Friday and returned on Mondays).
Special Blankets, Cuddles, and Pacifiers

Most children are comforted by meaningful objects at home and they are encouraged to bring something to the center that has a special meaning. Please make sure that these items are labeled and taken home (if appropriate) to be laundered.

Needs and Services Plan

Infants' and toddlers' needs change rapidly. You will be asked to update your child's Needs and Services Plan quarterly so that we know of any changes.

Nutrition

Parents provide lunch and all snacks for their child. Refrigerators and microwaves are available. Kids on Campus teachers will assist you in labeling and dating all bottles, which is mandated by State licensing requirements. Individualized, prepared bottles must be brought daily, not in one large container. We are not allowed to accept frozen breast milk. Once your child starts drinking regular milk please bring in a small carton for him/her.

If your child will be at KOC for lunchtime, please send a nutritious lunch (sandwich, fruit, crackers, cheese, soup in a thermos, microwaveable foods, etc.)

Please do not send foods that could cause choking, such as:

  • Peanuts and/or whole nuts
  • Whole grapes
  • Honey
  • Fruit with seeds and or tough skins not peeled
  • Popcorn
  • Chunks of hot dogs

Please inform the center, in writing, of any food restriction and allergies. Our staff will work with you to ensure your child is served food that is safe for him/her. Teachers will post a list of children with allergies or restrictions to remind them and others working in their room.

PLEASE DO NOT SEND CANDY OR GUM TO SCHOOL!

Do not leave medicine or vitamins in your child's lunch box!
