Health Insurance 2009-2010

 

TO ALL FALL 2009 UNDERGRADUATE STUDENTS, THE HEALTH INSURANCE WAIVER DEADLINE DATE HAS BEEN EXTENDED TO SEPTEMBER 21, 2009.

 


Santa Clara University has instituted a MANDATORY health insurance requirement for: 

  1. All new Incoming Undergraduate Students
  2. Continuing Undergraduate Students
  3. All International Students holding an F1 Visa (this includes undergraduate, graduate, and law students)


UNDERGRADUATE STUDENTS: 

All Undergraduate Students must either enroll in the University sponsored insurance plan or show proof of your own health insurance plan by completing the 2009-2010 waiver form by the deadline date The waiver will be accepted if the student’s insurance is comparable to the University sponsored plan.  If you do not submit an acceptable waiver, you will be automatically enrolled in the University sponsored plan and your student account will be charged $1398.00 for the annual insurance policy.   EXTENDED DEADLINE DATE: SEPTEMBER 21, 2009

GRAD STUDENTS: 

F1 VISA International Students must either enroll in the University sponsored insurance plan or show proof of your own health insurance plan by completing the 2009-2010 waiver form by the deadline date. The waiver will be accepted if the student’s insurance is comparable to the University sponsored plan. If you do not submit an acceptable waiver, you will be automatically enrolled in the University sponsored plan and your student account will be charged $1931.00 for the annual insurance policy.  EXTENDED DEADLINE DATE: SEPTEMBER 21, 2009

For all other Grad Students enrollment into University's sponsored insurance plan is voluntary.

LAW STUDENTS:

F1 VISA International Students must either enroll in the University sponsored insurance plan or show proof of your own health insurance plan by completing the 2009-2010 on-line waiver form by the deadline date. The waiver will be accepted if the student’s insurance is comparable to the University sponsored plan. If you do not submit an acceptable waiver, you will be automatically enrolled in the University sponsored plan and your student account will be charged $1661.00 for the annual insurance policy.   EXTENDED DEADLINE DATE: AUGUST 24, 2009

For all other Law Students enrollment into University's sponsored insurance plan is voluntary.
