    Use this form whenever your student organization is planning any event or activity with a particular level of risk. Examples of such events and activities include (but are not limited to):

    • Sports
    • Physical activity
    • Any trip or off-campus activity

    Each person participating in the event or activity needs to review, complete all sections, and sign at the end.

    Use this form whenever your student organization is driving cars or vans to any event or activity off campus. Each person who is driving needs to initial at section 2 and sign at the bottom.

    We no longer require drivers to attach photocopies of their driver's license, auto insurance card, and auto registration. The expectation is that the driver has valid driver documents (i.e. not expired). Remember, it is against the law to drive with expired documents!

    Also, if some or all of the drivers have filled out this form for another trip earlier in the academic year, then they do not need to do this again.

    >>> Media Release Form & Cover Sheet (PDF)

    Use this form whenever your student organization plans to use any photos or videos for promotional use (or that will be posted publicly such as on a website or social media). Each person in the photo or video must review and sign the Media Release Form.

  • iconReview of Forms

    Your CSL advisor will review all submitted forms to make sure requested information are received. For events and activities, it is recommended that these forms are submitted to your advisor at least 3 business days in advance. This will give your advisor time to review the forms and contact you if there is any missing information that needs to be submitted by anybody participating in the event or activity.
