If your permit is lost or stolen at any time, just fill out the following form and report to Parking & Transportation Services at a $15 replacement fee.
If your original permit is found, it is invalid and must be turned into Parking & Transportation Services. If the reported permit is found in use, the permit will be confiscated and permit privileges will be revoked.
