General Information
There is information that all new students are required to fill out online using the Cowell Center My Student Health Portal from the MySCU Portal. Information includes:
- Immunization record
- Medical history
- Health insurance
Immunization Record
New students are required to provide immunization information online using the Cowell Center My Student Health Portal from the MySCU Portal
Medical History Form
New students are required to provide their medical information online using the Cowell Center My Student Health Portal from the MySCU Portal.
Health Insurance Waiver
Santa Clara University requires all students to have health insurance. Students with their own health insurance must complete the online waiver form by the established deadline. Students not completing the waiver form by the deadline will be automatically enrolled in and charged for the SCU-sponsored health insurance plan.
IMPORTANT: The waiver form is required each academic year.
Deadlines
For students beginning enrollment in September 2023:
- Immunization Record: August 1, 2023
- Medical History Form: August 1, 2023
- Health Insurance Waiver (priority deadline): July 18, 2023
- Health Insurance Waiver (final deadline): September 22, 2023
Note: Enrollment in 6 units required to access these forms.
For students who deposit after August 1 to confirm their enrollment at SCU, the deadline for the Immunization Record and Medical History Form is 10 days after submitting the deposit.
Students beginning enrollment in January 2024 will receive an email from the Cowell Center about requirements and deadlines.
