Official transcripts for the Jesuit School of Theology are handled by JST's Registrar, Jim Oberhausen.
Transcript requests must be made in writing. Please fill out, sign, and submit the transcript request form. You can return the form in one of the following ways:
- Attach a scanned copy of the signed PDF form in an email to Jim Oberhausen: firstname.lastname@example.org
- Fax the form to (510) 841-6476 in care of Jim Oberhausen, JST Registrar.
- Print and send the form via mail:

The charge is $5.00 USD per transcript. Payment is only accepted in cash, check, or money order, under separate cover if you choose to fax or e-mail the request form. Checks should be made out to "JST" with "transcript" noted on the memo line of the check, if there is one. Payment is not required in advance, though timely payment is greatly appreciated.
