Registration Forms

All forms must be submitted to the Graduate Office in Guadalupe Hall, 3rd Floor, Room 330. Please refer to the Academic Calendar for all deadlines of adding ad dropping classes. No late adds or drops will be accepted.

  • Needs signature of instructor and department chair

PERMISSION TO TAKE GRAD/UGRD CLASS FORM

  • Needs signature of Advisor
  • Undergraduate Students must have department chair signature
